I am on the fourth try this year to breed my 9 yr old maiden mare qh through AI. The first try we may have missed her ovulation by a couple hours but bred anyways. The second try we hit her cycle perfect! Bred her two days in a row gave her te injection to ovulate the first day. The thrid time the timing was perfect as well but we gave her oxytocin a couple times and even flushes her uterus in case the fluid was causing a problem. We thought for sure she would take. When we checked her with ultrasound she was back in heat on the right side for the fourth time! Could there be something wrong with her right ovary? We dis culture in the beginning. And is it strange she is not coming in on the left at all? We are letting her have this cycle then we are going to short cycle her and see if she comes in on te left then breed number 4. Any comments/thoughts?

What did the cytology smear show? You did have a cytology smear prepared and read in conjunction with the culture didn't you? Many vet's don't do it even though they should, and a culture alone is worthless - read that linked article for more details.

The fact that she is producing follicles on one ovary more than the other means nothing.

I don't know if one was done but she is a maiden mare so if not than maybe that's why.

Preparation and reading of a cytology smear is part of the uterine culture process - if you are using knowledgeable reproduction veterinarians. If a culture is done, but not a cytology smear - you're not.

I've seen maiden mares with a Caslick's procedure in place have a Pseudomonas aeruginosa infection in the uterus, so being a maiden mare means nothing. Which is a moot point anyway, as you already observed that there was a culture swab taken.
