Young warmblood mare was showing strong heat and was palpated today. The vet said she had a size 40 follicle that is softening. He also was surprised at the "heaviness" of the utereus. She's only had one foal and was left open because of the lateness of her '00 foaling. Vet took a culture. Any ideas what might be going on?

You might want to evaluate this mare's uterus with ultrasound to rule out the possibility of excessive uterine fluid.
Generally though during estrus (which with a 4 cm follicle is likely the stage she was at), the uterus becomes more vascular and the wall thickens, so the "heaviness" (not a very helpful description - was something more accurate offered?) may not be an unusual situation for this mare.
