Possible penis injury please help

Postby Kleonaptra » Sat Mar 16, 2013 7:20 pm

Hi all. Im extremely worried about my boy right now and any advice would be welcome.

In late Oct my boy started breeding his mare. After the initial matings they were turned out together and he overdid it quite a bit. I removed him as he obviously had a sore penis - red and quite flaccid, with a bend in the middle - however he was still able to pee, drop it down, suck it up and began to maturbate again after only a day away from the mare. (masturbating is one of his favourite activities and would usually do it frequently throughout the day)
I put them together again and for the next two months she came in season and he was breeding her, all very orderly. He was maturbating and peeing and breeding just fine. In her season early Dec I suspected he had gotten her pregnant (with a little help from me and a tranquil paste - she was not standing still for him to finish inside her) as by the end of Dec she had not returned to season.
By now the mare has shown enough differences of behaviour that I am satisfied she is pregnant. However its been non stop trouble for my boy.
Late Dec - nail in foot after escaping paddock. Hoof capsule only just penetrated, dug out and cleaned and poulticed for 3 days. Short course of sulprim. Returned sound after only 2 days, healed without incident.
Late Feb, presented with mild cold, cough, nasal discharge and temp of 39.9(celsius) was treated with four days AM and PM sulprim, bute was recommended but I did not use it unless his temp was up, due to him being seperated from the girls I did not want him running around.
Cough persisted but he went back to work for about a week, then presented with abcess in left nostril. Vet was called, abcess cleaned with salt water and drained. Not necessarily related to his respiratory infection, lungs were better but glands swollen and congested in sinus. 4 days of penicillin and bute. I suspected nasal inflammation from a head injury when he was 2yrs old, causing swelling occassionally over right eye and discharge from right eye and nostril. Vet agreed improper draining of nasal secretions could be the cause. Improved visibly, cough disappeared, back to work just last Wed, 13/3
Presented last night mild colic. Walked him for half hour til passed manure. I had been at work all day and my partner had failed to notice any changes or that he had gorged himself on water. Some gut sounds by 2am, I went to bed.
This morning swollen testicles and extremely distressed. Called vet immediately, by the time she arrived he had managed to pass some urine and had no swelling of scrotum. Was given gentomyacin, bute and penicillin. Bloods taken, test results back tomorrow will determine treatment.
Ive realized he only masturbates now straight after he's had bute, and it used to be something he did all day. He pees often, covering the scent of the mares and their manure, but doesnt seem to pee as much as he used to. Im suspecting something wrong there, but Im not sure how it ties in with everything else. My vets are amazing and over the last few weeks have gotten very fond of my boy. The amount of times Ive heard 'He's so good for a stallion!' cannot be counted, they all have great bedside manner and its one of the best equine clinics in the state. Today his temp was quite high but he has an appetite and is only a little out of sorts.
He has not been right since we lost his mate to a broken leg last september. He still sleeps on the grave. I just dont know what to do for him in that regard, if his emotional state is affecting his immunity, Ive done all I can by getting him a mare.
Thanks for listening guys, any ideas are welcome. Thanks

Stallions masturbate every 90 minutes on average, so there's nothing unusual in that part of your stallion's behaviour.

I am perturbed at the apparent wide use of non-specific antibiotics. While the use in the case of the nail in the foot is valid, most coughs are viral in nature, and therefore not susceptible to antibiotics. Overuse of antibiotics can lead to development of resistant organisms as well as upset of the gut flora of the animal. This in turn can lead to difficult-to-resolve infections and colic situations.

Testicles don't swell up and down in the space of a few hours. While the swelling may be initiated in a few hours, it's not going to dissipate in that same time frame unless perhaps it's as a result of an allergic reaction (bite, sting etc,). Once again, antibiotics were given in an undiagnosed situation, which is bad.

Overall, things just aren't adding up here. I'd recommend taking him to a veterinary teaching (University) hospital or a specialist equine center where he can be monitored over a period of time.

I have to disagree that 39 is a normal temp. Normal range in CELSIUS degrees is 36 to 38. Anything over 38 is warning, temps of 40 plus can kill. I assume you are in America?

Penicillin is a pretty standard use in Australia. I am using a specialist equine clinic - Agnes Banks equine clinic is one of the best. When I worked on a stud down south one of our borders took his mare out here everytime for her AI - over an hour drive. They have a great reputation.

His bloods came back and there is a definite infection. He now has a large lump on his left testicle, he's having trouble masturbating. I know its a normal behaviour, thats why I made a point of mentioning he has not been doing it. Yesterday I observed him masturbating just before feed time, after he ejaculated he galloped around shaking his head like "Oh, that felt good!" I think its the anti inflammatory - I have kept him on butazone morning and night. He still pees but not as much. Theres definitely some pressure on his urethra in my opinion.

The vet cant come until tomorrow while his temp is stable. They are booked up. If he gets into an emergency before that I will call them again. I have a dr's appt for myself this afternoon - already rescheduled 3 times - so I wont be able to watch him and Im just frantic over that. He seemed well this morning and not too down. Good appetite, good temp, moving around well. I was beginning to get worried about EHV1 but now with the lump on his testicle Im really hoping for a best case scenario, if he cannot be healed of possible cyst/abcess, hopefully we can get out of this only losing one.

You're correct. 39 is not normal. I miscalculated my conversion.

As noted, testicles don't swell and decrease swelling to nothing in a few hours unless there's a bite/sting or something of that ilk. As you are indicating that there is now a lump, it is suggestive of orchitis. Have you looked at the semen? Evaluated it for WBC content? It might be a good step in the diagnostics.

I still think continued evaluation/treatment at a veterinary teaching (University) hospital or a specialist equine center might be a good move.

Good luck.

Thanks for your replies Jos, I do appreciate your expertise and you taking time to reply.

I was so concerned I rearranged my schedule to have the vet come today. The left testicle (with the lump) did not reduce fully (sorry if I didnt make that clear) but after urination or masturbation reduced closer to normal. We gave him a muscle relaxer and checked him out thoroughly, no sign of blockages in the penis or urethra. The vet said swelling in the testicles may be glandular given the severity of his infection. If it was torsion or blockages there she thought the swelling would be worse and he would show much more pain. He is now on Doxylycine Am and Pm, if the swelling does not begin to reduce in 24 hours he will have to go to the clinic for ultrasound. I asked her if I should just make arrangements today to take him in (we dont have a float or truck, only have 3 horses) she said give him the 24 hours. I have upped his bute to 10mls Am and Pm as on the anti inflammatory he has been peeing more and drinking more which I feel is vital to clearing the infection.

We also checked his mare who has been a bit down today and had temp of 38.8 this morning. She is steady in the 38 range. I am going to monitor her carefully and if her temp spikes I'll start bute and get her on antibiotics as well. The vet said its possible she has brought in a virus - we got her in Oct and my horses do not have contact to outside horses so anything unknown would have a greater affect on them. The old girl (20 this year) is fine, showing no increased temp but she is not as 'touchy feely' as the other two thus would not have had the same close contact with the mare he had, even though they run together she is usually off on her own while the other to are always together, often touching.

EHV1 is a possibility and we will begin vaccinating at 5 months as recommended. The vet said any virus that he may (or may not) have been infected with is no longer the issue, the secondary infection he is having trouble beating IS the issue so if his improvement is not significant in that 24 hr period he is going to the clinic. He is a young healthy boy of 7 so having immune impairment of this magnitude is a huge worry for me. I will look into orchitis and mention it to the vet, thanks

Was the nasal discharge of the February abscess cultured?

I might suspect Strangles progressed to bastard Strangles if it hasn't been looked at already.

Unfortunately no, we did not culture because we assumed he only had a cold or minor sinus infection due to his previous head injury. At the second visit strangles was discussed but the vet said his symptoms didnt line up exactly for a strangly horse.

The mare is fine today, yesterday she had a clear discharge from her nose, today nothing. Her temp is back down to 37.6

This morning his temp before medication was 37.5, a great sign. The left testicle is still a bit swollen with the fluidy soft lump still there. It looks better than yesterday but still not right. He is much happier, moving around a lot more and bothering his girls. Masturbated twice this morning and seems very relieved, no more stress lines around his muzzle and eyes. He is still walking carefully, like he's keeping his back legs apart to not brush his boy bits. Im encouraged but still worried. She said if the swelling goes down in 24 hours but his improvement is not marked by friday he will still be going to the clinic so fingers crossed he keeps progressing like this.

Its my first season and his so Im pretty new to it. At the stud I worked at I was more involved with mares and weanlings, I was also the show groom. My stud boss had a thing about women and stallions - I could groom them and clean their yards or stables but was not allowed to exercise them or be involved with any procedures if one had problems. I helped with semen collection a few times. Stud boss also had an issue with women showing, I could show prep but not show. I proved him wrong by competing with my gelding and winning multiple shows. I went out on my own about 7 years ago when my boy was born because I knew I would breed with him and my boss didnt want another stallion on the property. Throughout his 7 years my boy has had plenty of injuries and illness' and I was always content if he couldnt compete at least I would show his foals, so this is the first time his breeding future has been at risk.

Just did a bit of research on testicular swelling and my boy looks NOTHING like that. Lots of pics for torsion, orchitis, abcess/cyst heamatoma etc and all involve hard hot swelling that is extremely painful, my boys is very soft and fluidy. Cant find much info on testicles being involved in glandular swelling due to infection.

With the nasal abcess we have a bit of african thorn around that Im working on getting rid of, the vet said the abcess was more likely caused by a thorn than connected to his infection. Him suffering the mild colic on Sat is concerning me as it seems a common symptom for testicle problems.
